Quick answer
Folic acid reaches the blood within 15 to 30 minutes, but building up your stores takes months. In trials, plasma folate leveled off after about 3 months of a daily dose, while red blood cell folate was still rising at 24 weeks and is estimated to level off at about 40 weeks.1,2,3,4
Key facts
- Serum folate mostly reflects recent intake, while red blood cell folate shows longer-term status.5
- Red blood cell folate has an estimated half-life of about 8 weeks.4
- US advice is to start folic acid at least a month before pregnancy.5
- Blood folate falls quickly after supplements stop, and faster after higher doses.2
The first hours
After an oral dose, folic acid appears in the blood within about 15 to 30 minutes and usually peaks within an hour.1
That quick rise is absorption, not a full tank. The body holds 15 to 30 mg of folate, about half of it in the liver, and topping that up takes weeks of daily intake.2,5
Weeks to months: what blood tests show
With the same dose every day, plasma folate levels off after about 3 months.2,3
Red blood cell folate, the longer-term measure, was still rising after 24 weeks of a daily supplement. Modeling puts its half-life at about 8 weeks and the time to a steady level at about 40 weeks.3,4
| Measure | What it reflects | Time to level off in trials |
|---|---|---|
| Plasma or serum folate | Recent intake | About 3 months |
| Red blood cell folate | Longer-term status | Still rising at 24 weeks; estimated at about 40 weeks |
NIH counts serum folate above 3 ng/mL and red blood cell folate above 140 ng/mL as adequate. Serum folate swings with recent meals and supplements, which is why red blood cell folate is the better guide to intake over time.5
Does a bigger dose work faster?
In the 6-month trial of 1,108 women, plasma folate rose further with larger daily amounts: by about 108% on 100 mcg, 259% on 400 mcg and 460% on 4,000 mcg. A single 4,000 mcg dose taken once a week raised it by 142%. The higher doses also fell away faster once they stopped.2
So a larger dose lifts blood levels higher, but red blood cells still need months to catch up, and anything above 1,000 mcg a day goes past the adult upper limit.4,6
Does methylfolate work faster?
After one dose, methylfolate peaked in the blood sooner, and higher, than folic acid in 24 women with and without the MTHFR variant. Over 24 weeks, a daily 416 mcg of methylfolate lifted red blood cell folate further than 400 mcg of folic acid, yet neither group had leveled off when the trial ended.3,7
In men
The timing trials above were done in women. None of the sources we use report separate timing for men, who have the same recommended amount of 400 mcg DFE a day.6
Alcohol interferes with folate absorption and speeds its loss. In healthy men, 8 ounces of red wine or 2.7 ounces of vodka a day for 2 weeks lowered blood folate.5
Planning a pregnancy
Low folate early in pregnancy raises the chance of a neural tube defect. The neural tube closes 21 to 28 days after conception, often before a pregnancy is known, which is why the advice is to start folic acid at least a month ahead.5
The authors of the 24-week trial suggested starting more than 4 weeks before conception, because red blood cell folate was still rising when their trial ended.3
If you are taking folic acid for a low level
If a doctor found low folate, ask when it will be rechecked. Low vitamin B12 can cause the same blood changes, and folic acid alone does not stop the nerve damage that low B12 causes.1,5
Questions people ask
Will I feel different after starting folic acid?
Not necessarily. Most people in the US already get enough folate, and what trials measure is the change in blood tests, not how people feel.5
