Quick answer
There is no official elderberry dose. Trials used 600 to 900 mg a day of a capsule extract, 15 mL of syrup two to four times a day, or an extract supplying 500 mg of anthocyanins a day. Most courses lasted 2 to 16 days, and the longest in healthy people ran 12 weeks.1,2,3
Key facts
- NIH lists one 15 mL syrup dose as 5,700 mg of elderberry extract.1
- The syrup and lozenge trials lasted 2 to 5 days.1,2
- Fresh European elderberries hold about 391 to 806 mg of anthocyanins per 100 g.4
- Neither NIH nor NCCIH gives a recommended daily amount.1,5
Amounts used in trials
| Form | Amount | How often | How long |
|---|---|---|---|
| Capsules of 300 mg extract | 600 mg a day, then 900 mg a day | 2 or 3 capsules a day | About 15 days |
| Syrup | 15 mL a dose | Twice a day for children aged 5 to 12, four times for others | 3 to 5 days |
| Lozenges of 175 mg extract | 700 mg a day | 4 times a day | 2 days |
| Capsules of an anthocyanin-rich extract | 500 mg of anthocyanins a day | 2 capsules twice a day | 12 weeks |
| Capsules of spray-dried juice, 10% anthocyanins | 1.2 g a day | 400 mg three times a day | 2 weeks |
| Infusion (tea) | 200 mL | Not stated in the review | 30 days |
Trials lasting 3 to 5 days gave 15 mL of elderberry syrup four times a day to adults and older children, and twice a day to children aged 5 to 12.1,2
In a small study without a placebo group, 22 healthy volunteers drank a 200 mL elderberry infusion for 30 days.2
Why the milligrams are hard to compare
A label may give the weight of an extract, the volume of a syrup or the amount of anthocyanins, and these are not interchangeable.7,8
Products differ in strength. In two small studies, 30 mL of a commercial elderberry extract supplied 147 mg of anthocyanins in one case and 278 mg in the other.7,8
Without the extract ratio or the anthocyanin content, two products with the same number of milligrams can differ widely. NIH also notes that some products sold as elderberry are diluted or adulterated.1
How long courses lasted
The trials in people who were already unwell gave elderberry for 2 to 16 days. The longest trial in healthy people that we found ran for 12 weeks.1,3
The 12-week trial in healthy women is the longest in healthy people that we found, and the liver and kidney blood tests in it stayed within normal ranges. We found no trial of longer daily use in healthy people.3
Do not dose with raw berries
Raw or unripe elderberries, and the leaves, stems, bark and seeds, contain substances that can release cyanide and cause nausea, vomiting and severe diarrhea. Cooking destroys them, so cooked fruit and properly processed products do not carry this concern.1,5
In 1983, 11 of 25 people at a gathering in California fell ill within 15 minutes of drinking juice made from wild elderberries crushed with their leaves and stems. The eight most affected had nausea, vomiting, stomach cramps and weakness and were flown to hospital, and all recovered.9
What's in ours
What’s in ours
- Immune Support: Elderberry Extract, 500 mg per serving, from the label
Our label gives the weight of the extract but not its anthocyanin content or extract ratio, so it cannot be compared directly with the trials above.
Who should ask before taking any amount
No upper limit has been set for elderberry. The longest trial in healthy people that we found gave an extract supplying 500 mg of anthocyanins a day for 12 weeks, and most others lasted 2 to 16 days.1,3
Questions people ask
How much elderberry per day for adults?
Adult trials used 600 to 900 mg a day of one capsule extract, 15 mL of syrup up to four times a day, or an extract supplying 500 mg of anthocyanins a day. None of these is an official dose.1,2,3
How many mg of elderberry per day did trials use?
Capsule trials used 600 to 900 mg of extract a day, but milligrams of extract are not a fixed measure. Products differ in how much anthocyanin each milligram holds.1,7,8
How much elderberry can children take?
Some short trials included children aged 5 to 12, at 15 mL of syrup twice a day, but the sources we use give no dose for children. Ask a pediatrician first.1
