Quick answer
It depends on what you measure. In single-dose studies, differences on mental tasks appeared from 1 hour after a 200 or 400 mg dose and were tracked for up to 6 hours. For tiredness, trials ran 4 to 8 weeks, and in pooled data, courses under 6 weeks showed no clear effect.1,2,3
Key facts
- In single-dose studies, people were tested 1 to 6 hours after taking ginseng.2
- Courses shorter than 6 weeks showed no clear effect on fatigue in a pooled analysis.3
- The ginsenoside Rb1 had a half-life of about 58 hours in one study.4
- Menopause trials lasted 12 to 16 weeks.5,6
Within hours: single-dose studies
In single-dose studies, people were tested from 1 hour to 6 hours after a dose, and differences from placebo were seen from the first test, 1 hour after the dose.1,2
Taking the same dose every day for a week gave the same short-term effects as the first dose, with no added gain and no fading.7
These were small crossover studies in young adults from one research group, and the effects were mixed: some answers became more accurate but slower. NIH's complementary health center, summing up the research, says the small amount of evidence on thinking points to possible effects in middle-aged adults but not in young adults.2,7,8
Over weeks: tiredness and menopause
Fatigue trials ran 4 to 8 weeks. In a pooled analysis, courses shorter than 6 weeks showed no clear effect on fatigue.3
In the 4-week tiredness trial, fatigue was scored at the start and after 2 and 4 weeks, and its main results were reported at 4 weeks. Even then, total fatigue scores did not differ from placebo; mental fatigue scores did.9
Trials in women after menopause lasted 12 to 16 weeks.5,6
So a fair test of ginseng for tiredness or menopause discomforts, as the trials ran it, means taking it daily for at least 6 weeks, and for menopause about 3 months.3,5
How long ginseng stays in your system
After a single 9 g dose of red ginseng extract taken at 8 a.m., the ginsenoside Rb1 peaked in the blood after about 9 hours. Compound K, which gut bacteria make from it, appeared after 4 to 9 hours and peaked after about 12 hours.4
Rb1 has a long half-life, about 58 hours in one study and 38 to 46 hours in another, against about 8 hours for compound K. None of the measured ginsenosides were left in the blood a week after a single dose.4,10
Half-life is the time it takes for the level in the blood to fall by half. In the first study, compound K could still be detected at low levels in 8 of the 10 men 36 hours after the dose. Both studies were small and gave young men large single doses of red ginseng extract, 9 g and 5 g.4,10
Morning or evening?
No study we found compared times of day. The 4-week tiredness trial gave one dose at 9 a.m. and one at 7 p.m., and the absorption study gave its single dose at 8 a.m.4,9
Before you start
If you take warfarin, ask before starting or stopping ginseng. The warfarin label says the INR should be checked more often when starting or stopping botanicals, and lists ginseng among those that may lessen warfarin's effect.11
Ginseng might lower blood sugar, and in one study single doses lowered blood glucose in healthy young adults within hours. If you have diabetes or take medicine that lowers blood sugar, ask your doctor first.1,8
Questions people ask
How long does Panax ginseng stay in your system?
Some ginsenosides linger for days. Rb1 had a half-life of about 58 hours in one small study, and none of the measured ginsenosides remained a week after a single dose in another. Compound K, made from Rb1 in the gut, peaked about 12 hours after a dose.4,10
